What this panel actually is

This is ZRT's Saliva Steroid LC-MS Profile 23: 18 endogenous steroid hormones, plus melatonin, plus the synthetic estrogen ethinyl estradiol found in most oral contraceptive formulations, plus three steroid synthesis inhibitors.

Why athletes look at more than testosterone

Training performance is influenced by far more than testosterone alone. Your ability to build muscle, recover from hard training, hold energy, regulate body composition, manage stress and sleep depends on how several hormone systems interact.

Most hormone tests answer one question: how much hormone is present. Testing both the upstream precursors and the downstream metabolites of the active hormones helps show which steroid synthesis enzymes are underactive, overactive, or blocked, whether by natural or pharmaceutical inhibitors. That is the difference between a level and a pathway.

What this test measures

Estrogen pathway. Estradiol (E2), estrone (E1), estriol (E3), ethinyl estradiol (EE).

Progesterone pathway. Pregnenolone sulfate (PregS), progesterone (Pg), allopregnanolone (AlloP), 17-OH progesterone (17OHPg).

Androgen pathway. Androstenedione, testosterone, dihydrotestosterone (DHT), DHEA, DHEA-S, 7-keto DHEA.

Glucocorticoid pathway. 11-deoxycortisol (11DC), cortisol, cortisone.

Mineralocorticoid pathway. Corticosterone, aldosterone.

Sleep marker. Melatonin.

Steroid synthesis inhibitors. Anastrozole, finasteride, letrozole. These are aromatase and 5-alpha-reductase inhibitors used in hormone optimization. Because they change how hormones are metabolized, knowing whether they are present changes how the rest of the report reads.

Why saliva

Saliva measures the bioavailable, unbound fraction of hormones, the portion actually available to tissues. ZRT describes this as relating more closely to symptoms of excess or deficiency than a total blood level does.

LC-MS/MS is the method ZRT uses for this profile. It measures 23 markers from one sample, including several that the standard five-hormone immunoassay panel does not report.
