Most cortisol in your bloodstream is bound to carrier proteins and isn’t available to your tissues. Saliva testing measures the free, bioavailable fraction — the cortisol that actually reaches and acts on cells — which is why it tends to relate more closely to symptoms of excess or deficiency. Saliva is also non-invasive and avoids the brief stress-related cortisol spike a needle stick can cause, and it’s well suited to collecting several samples across a single day at home.
Cortisol isn’t meant to be flat. In a healthy rhythm it peaks about 30 minutes after waking and then declines steadily, reaching its lowest point at bedtime. A single blood draw captures one moment on that curve. What does this test measure?
Bioavailable (free) cortisol in saliva, collected four times across one day to show your daily cortisol rhythm. It’s processed by ZRT Laboratory.
Does it include DHEA or other adrenal hormones?
No. This kit measures cortisol only, at four time points. It does not include DHEA-S or other hormones.
How is this different from a cortisol blood test?
A blood test usually captures a single moment, and most blood cortisol is protein-bound and unavailable to tissues. This test measures the free, bioavailable fraction at four times of day, so it shows the rhythm rather than one snapshot.
Is saliva cortisol testing accurate?
Salivary cortisol testing is a well-established method for measuring free (bioavailable) cortisol. Collecting samples throughout the day helps evaluate your daily cortisol rhythm rather than relying on a single measurement.
Why do I wake up at 3 a.m.?
Cortisol is normally at its lowest overnight. When the daily rhythm is disrupted, some people notice nighttime waking — though sleep, blood sugar, and stress all play a role too. A 4-point test shows what your cortisol is doing across the day and into the evening, which can be a useful piece of the picture to review with your provider.
Can stress permanently damage my cortisol rhythm?
“Permanent damage” isn’t the right way to think about it. The stress-response system (the HPA axis) adapts to ongoing demands, and rhythms can shift over time — but they can also shift back. Testing gives you an objective snapshot of where things stand now and a way to track changes as you make them.
Does cortisol affect menopause symptoms?
