Saliva testing is also not appropriate if you use sublingual or troche (lozenge or drop) hormones, because the hormone can contaminate the sample directly and produce falsely high saliva levels. ZRT states that blood spot testing is the preferred option for these patients. If you produce very little saliva, collection may be difficult. And this saliva panel does not include thyroid markers. If thyroid is part of your question, a combined saliva and blood spot profile fits better, and the Flourish team can point you to it.
Most of the sex and adrenal hormones in your bloodstream are bound to carrier proteins and are not available to your tissues. Saliva measures the free, bioavailable fraction, the portion that actually reaches and acts on cells, which is why ZRT describes saliva as relating more closely to symptoms of excess or deficiency. It is also non-invasive and collected at home. For women using topical creams or gels, saliva reflects hormone delivery to tissue better than standard venous blood does.

What hormones does this test measure?
Five, in saliva: estradiol, progesterone, testosterone, DHEA-S and cortisol, in their bioavailable (free) forms. It is processed by ZRT Laboratory.
Why measure five hormones instead of one?
Hormones work together, so symptoms usually reflect more than one of them. Testing a single hormone risks missing the pattern. Measuring five at once helps you and your provider see how they relate, instead of guessing which one to check.
Why do I suddenly feel different in my 40s?
Many women notice shifting energy, sleep, mood and cycles in their 40s as estrogen and progesterone begin to fluctuate during the years before menopause. A hormone panel will not by itself diagnose that transition, but it can show where your levels sit so you and your provider can interpret your symptoms with real data.
How do I know if I am in perimenopause?
Perimenopause is usually recognized by a cluster of changes, including irregular cycles, hot flashes, and sleep and mood shifts, often starting in the 40s, and it is a clinical assessment your provider makes. Hormone levels fluctuate during this time, so this panel is most useful as one input to that conversation rather than a stand-alone answer.
Can a hormone imbalance affect my sleep?
Sleep is influenced by several of the hormones in this panel, estradiol, progesterone and cortisol among them, so changes in any of them are commonly associated with disrupted sleep. Seeing them together can help identify what is worth discussing with your provider.
Can a hormone imbalance cause anxiety or mood changes?
Estrogen, progesterone and cortisol all interact with mood, and shifts in them are commonly associated with irritability, low mood and anxiety-like symptoms. This panel gives an objective look at those hormones. It does not diagnose a mental health condition.
Why has my libido changed?
Low testosterone and low estradiol are both commonly associated with reduced libido in women, and stress can play a role too. Because this panel measures all of them, it can help pinpoint which may be contributing.
Could an imbalance between estrogen and progesterone contribute to my symptoms?
Possibly. When estradiol is high relative to progesterone, people commonly report tender breasts, water retention, irritability and difficult cycles. This panel measures both hormones and their relationship, which is the way to understand whether that pattern fits you.
Can stress affect my estrogen or progesterone?
Stress hormones and sex hormones do not operate in separate silos. They interact. That is why cortisol is included alongside estradiol and progesterone here, so you can see them in context rather than in isolation.

Do I need to collect on a specific day of my cycle?
If you still have menstrual cycles, the timing of collection matters for an accurate read of progesterone. Your kit instructions explain when to collect, and the Flourish team can help if you are unsure.
Can I use this if I am on hormone therapy?
It depends on the delivery method. Saliva works well for monitoring topical creams or gels. It is not appropriate for sublingual or troche hormones, which can falsely raise saliva levels, and ZRT recommends blood spot testing instead. Ask the Flourish team if you are unsure which fits.
Should I test my hormones before starting BHRT?
A baseline before starting, and re-testing to monitor afterward, is a common approach. ZRT designs these panels both to assess baseline levels before hormone therapy and to monitor dosing. Whether and how to start any therapy is a decision for you and your provider.
Does this test include thyroid?
No. This saliva panel covers sex and adrenal hormones only. If you want thyroid markers too, a combined saliva and blood spot profile is the better fit.
How long until I get results?
Typically about two weeks after the lab receives your samples.
