Cortisol is not meant to be flat. In a typical rhythm it peaks shortly after waking, then declines through the day to its lowest point at bedtime.
A single morning sample gives you one number from the top of that curve. That is genuinely useful if you want a simple check, a baseline before you change anything, or a low-cost first look.

Most cortisol in your bloodstream is bound to carrier proteins and is not available to your tissues. Saliva measures the free, bioavailable fraction, which is why ZRT describes it as relating more closely to symptoms of excess or deficiency and as a good option for monitoring cortisol.
Saliva also avoids the brief cortisol rise a needle stick itself can cause, which matters when cortisol is the thing you are measuring.
