They are not independent systems, and ZRT's own data sheets say so.
Cortisol is the body's stress hormone. As part of the fight-or-flight response it temporarily shuts down processes that are not immediately needed, and ZRT names hormone production among them. In the male hormone data sheet the point is made directly: some practitioners find that testosterone therapy is of little benefit unless cortisol is addressed first, which is why ZRT recommends a complete hormone profile before starting any hormone replacement.
Looking at estradiol, progesterone, testosterone and DHEA-S while your cortisol is high, and not knowing your cortisol is high, is how people end up chasing a downstream number.
Cortisol is not meant to be flat. It normally peaks shortly after waking and declines through the day to its lowest point at bedtime. A single reading is one point on that curve. Four readings, taken at set times, show the shape.
The shape is where the useful pattern lives. Two people can have the same morning cortisol and completely different days.
Steroid hormones in blood are largely bound to carrier proteins and in that form are not available to your tissues. Saliva measures the unbound, bioavailable fraction, which ZRT describes as relating more closely to symptoms of excess or deficiency and as a good option for monitoring hormone therapy.
It is also non-invasive, collected at home, and well suited to taking several samples across a single day.
ZRT states that saliva testing is not appropriate for sublingual hormone users, because the hormone contaminates the sample directly, and that blood spot testing is the preferred option for these patients.

What does this panel measure?
Estradiol, progesterone, testosterone, DHEA-S, and cortisol at four time points across one day.
How is this different from the standard hormone saliva kit?
Same five hormones. This one measures cortisol four times instead of once, so it shows your daily cortisol rhythm.
How is this different from the 4-point cortisol kit?
That kit is cortisol only. This adds the four sex and adrenal hormones.
Why does a hormone panel include cortisol at all?
Because cortisol can suppress the production of other hormones as part of the stress response. Reading sex hormones without knowing what cortisol is doing can be misleading.
Do I collect everything on the same day?
Yes. One collection day, with the cortisol samples at set times. Full instructions come in the kit.
Does it include thyroid?
No. ZRT's saliva profiles do not include thyroid hormones.
Can I use this to monitor hormone therapy?
